Use of rotational atherectomy for treatment of in-stent restenosis with coronary double-mesh stent underexpansion. Case report

چکیده
Treatment of in-stent restenosis lesions, especially calcified with stent underexpansion, generally requires more complex techniques, such as rotational atherectomy. The case reported is a male patient 99% focal lesion at the origin first diagonal branch, where two stents were implanted 14 years ago. After failure balloon angioplasty alone, ablation plaque and part struts was performed using atherectomy technique, which allowed implantation new totally expanded.

Rotational atherectomy for the treatment of in-stent restenosis.
Restenosis after coronary stent implantation remains the major limitation of this treatment modality. At present, re-dilatation is considered the therapeutic option of choice for focal lesions, however, long restenotic lesions (>10 mm) do not respond favourably. Despite the emerging concept of intracoronary radiation, encouraging acute procedural results are also reported for different debulkin...
متن کاملDirectional coronary atherectomy vs. rotational atherectomy for the treatment of in-stent restenosis of native coronary arteries.
Management of in-stent restenosis has become a significant challenge in interventional cardiology. Since the mechanism of in-stent restenosis is predominantly intimal hyperplasia, debulking techniques have been used to treat this condition. This study is a nonrandomized comparison of the immediate and long-term results of directional coronary atherectomy (DCA; n = 58) vs. high-speed rotational ...
متن کاملRotational atherectomy does not reduce recurrent in-stent restenosis: results of the angioplasty versus rotational atherectomy for treatment of diffuse in-stent restenosis trial (ARTIST).
BACKGROUND Aim of this trial was to compare rotational atherectomy followed by balloon angioplasty (rotablation [ROTA] group) with balloon angioplasty (percutaneous transluminal coronary angioplasty [PTCA] group) alone in patients with diffuse in-stent restenosis. METHODS AND RESULTS The ARTIST study is a multicenter, randomized, prospective European trial with 298 patients with in-stent rest...
متن کاملRotational atherectomy of right coronary artery complicated with stent dislodgment and ostial dissection – case report
Highly calcified lesions in coronary arteries are a frequent cause of percutaneous coronary intervention failure. In such cases rotational atherectomy seems to be an efficient alternative that allows facilitation of the calcified atherosclerotic plaque and ensures successful treatment. We present a case of rotational atherectomy of the right coronary artery complicated with stent dislodgment an...
متن کاملOverview of Intracoronary Brachytherapy for in-Stent Restenosis of a Drug-Eluting Stent
Percutaneous coronary intervention with stenting is considered recently as the most common procedure for the treatment of symptomatic coronary. The article reviewed 41 studies published during 1997-2019 on intracoronary brachytherapy for in-stent restenosis of a drug-eluting stent. Intracoronary radiation therapy was finally confirmed in the setting of in-stent restenosis using as adjunctive th...
